 HERE’S HOW IT WORKS:

1. Fill out a manual entry form at any adoption day (we love visitors) or click here and mail your form to Katz 4 Keeps or fill out an online form at www.birdiesforcharity.playerschampionship.com

2. Make a donation to Katz 4 Keeps (minimum donation is $20.00) If you donate more then $20 please complete a separate form for each $20. This will maximize your chance of the Grand Prize and ours.

3. Guess how many birdies will be made during the competitive rounds of the Players 2012 Championship

4. Guess a tiebreaker question (how many balls will be hit in the water at #17 at the Players 2012 Championship)
Remember:

• Your donation is tax deductable

· If you win the Grand Prize Katz 4 Keeps will receive a matching prize

· Katz 4 Keeps will be eligible to participate in a bonus pool from the Players Championship
